Through this partnership, AZ Blue will use Monogram’s extensive network of in-home providers and support services to improve the care of their members.

This unique partnership will give this population across eight counties in Arizona access to Monogram’s suite of specialized clinical and support services including evidence-based clinical pathways and protocols, in-home specialty care, complex case and disease management, and comprehensive medication therapy management.

NCQA awarded the accreditation following its detailed review of Monogram’s case management services for alignment with evidence-based care pathways, processes, and service delivery. This achievement highlights Monogram Health’s commitment to being a clinical leader for patients with polychronic diseases and supporting individuals with complex chronic kidney and end-stage renal disease through innovative home-based care services.

“Patients experiencing chronic kidney or end-stage kidney disease usually have multiple underlying chronic conditions. They’re taking numerous medications and may be dealing with challenges such transportation concerns, access to healthy foods or difficulties managing medical appointments,” said Katrina Cope, Senior Vice President, Strategic Initiatives and Implementations for Monogram Health. “Our complex case management program focuses on the whole person – not just the diagnosis – and ensures that patients and their families receive the information, education and treatment they need to make informed decisions that can meaningfully impact their health and quality of life.”

Former HCA Healthcare VP named to newly created Chief Growth Officer role 

NASHVILLE, Tenn. Dr. Shah brings nearly two decades of executive and health care experience to the role, where he will lead the continued rapid and widespread growth of Monogram’s innovative in-home care delivery model. 

“With his impressive background in physician alignment, patient acquisition, strategy and value-based contracting, Dr. Shah is uniquely positioned to direct the continued expansion of our geographic clinical footprint as well as growing our portfolio of value-based client partners,” said Monogram Health CEO Mike Uchrin. “We are delighted to welcome a third physician executive to our leadership team as we drive physician-led, in-home value-based care to improve access, outcomes and affordability for our patients.”

Prior to joining Monogram, Dr. Shah served as corporate vice president for strategy and product innovation for payer contracting and alignment at Nashville-based HCA Healthcare. Prior to that, he was president/CEO of Provision Health Partners and president of Provision CARES Nashville. Early in his career, Dr. Shah also served as regional medical director for the Texas Department of State Health Services and as chief physician of preparedness for the City of Houston.

Dr. Shah earned his medical degree from UT Health San Antonio and received his juris doctorate from the University of Houston Law Center. As a board-certified OBGYN and a fellow of the American College of Obstetricians and Gynecologists, Dr. Shah has practiced in public, private and academic settings. In 2010, Dr. Shah became the first-ever executive fellow at Texas Children’s Hospital Integrated Delivery System, where he worked on enhancing system quality, developing service lines and improving health plan operations.

Monogram Health Adds Bhavesh Shah to Executive Leadership Team

NASHVILLE, Tenn. Shah brings two decades of financial leadership and growth stage executive experience to the role, where he will lead Monogram’s financial operations, including actuarial and medical economics, treasury, financial planning and analysis, and audit.

Before that, he spent more than eight years leading financial operations for Fortune 500 health care companies, including roles as vice president of FP&A at CVS Health’s retail business, and vice president and CFO of Aetna’s consumer health products and services business where he oversaw Aetna’s value-based care and complex care management programs. Earlier in his career, Shah served as a health care investment banker at Lehman Brothers and Barclays Capital. He also served five years in the U.S. Army, rising to the rank of Captain. Shah holds an MBA from the Massachusetts Institute of Technology and a bachelor’s degree in engineering from the University of Connecticut.
