Care Team

Your Care Team is dedicated to visiting and supporting you in your home. You will be paired with a registered nurse care manager and a social worker for home visits; they are guided by the leadership of your kidney doctor (nephrologist). Your Care Team is further supported behind the scenes by caring professionals such as cardiologists, internal medicine specialists, geriatricians, dieticians, care coordinators, pharmacists, and additional social workers.

Like other front-line healthcare workers, our team members are taking extra precautions — all in line with CDC guidance — to ensure you remain safe. This includes wearing personal protective equipment (PPE), remaining at least 6 feet apart and washing and sanitizing their hands once they arrive inside your home. If you remain uncomfortable with someone entering your home, we can schedule virtual telehealth or telephonic visits.

Support

We help you keep track of medications and appointments, arrange transportation if needed, and access additional services as necessary to ensure you stay as safe and healthy as possible. We also communicate and coordinate with your personal primary care physician and other specialists.

Medical Care

Did you know that over 40% of people with chronic kidney disease don’t even know they have it?

At Monogram, we focus on helping you understand your health, your doctors, and your medications. If you already have a primary care physician and nephrologist, we look forward to engaging them. However, if you do not have a nephrologist, that’s OK. Our health care professionals will visit you in your home and connect you to our team of specialists to treat your chronic conditions, adjust your medications, if necessary, and help you make the most informed decisions possible about your specialized kidney and primary care pathway.

Patient Stories

Prior to her diagnosis, Ann* had no idea she was suffering from late-stage chronic kidney disease. When her health plan referred her to Monogram Health, we immediately sent a nurse care manager to meet with Ann in her home to discuss the impact of her declining kidney function. In discussing treatment options with her nephrologist, Ann became interested in transplantation as a care pathway. Monogram’s Medical Director met with her nephrologist to discuss a plan of care; Ann was subsequently approved for a kidney transplant by her health plan, and her daughter has been identified as a living donor candidate.
