If you don’t have a nephrologist, that’s okay—our providers will visit you at home and connect you with our team of specialists. We’ll treat your chronic conditions, adjust your medications if needed, and help you make informed decisions about your specialized care pathways.

Ann’s Story
Prior to her diagnosis, Ann* had no idea she was suffering from late-stage chronic kidney disease. When her health plan referred her to Monogram Health, we immediately sent a nurse care manager to meet with Ann in her home to discuss the impact of her declining kidney function. In discussing treatment options with her nephrologist, Ann became interested in transplantation as a care pathway. Monogram’s Medical Director met with her nephrologist to discuss a plan of care; Ann was subsequently approved for a kidney transplant by her health plan, and her daughter has been identified as a living donor candidate.
